ATTACHMENT A SPECIAL REPORT TO LJAA
This form is to be used to report any matter concerning LJAA activities or contests that merits the attention of the Association. It shall be used to report phases of the athletic program which the supervisor should immediately attempt to improve. It may be used to report an exceptionally good job of game administration as well as all instances involving unsportsmanlike conduct on the part of coaches, players, or fans should be reported on this form. Officials are expected to report problems within 5 days so the League may take prompt action.

The Association must receive report of action within five days. This report needs to be submitted to an executive board member (President, VP, Secretary or Treasure).
